All forms are now available and made accessible through the State of Montana's Court system website at https://courts.mt.gov.

Forms are also available in hard copy at the Missoula Self Help Family Law Center located at 200 West Broadway in the District Courthouse Room 271. The Center is open Monday through Thursday from 8:30 AM to Noon and 1:00 PM to 4:00 PM. There are also forms and information available through https://www.montanalawhelp.org/ or courts.mt.gov.

The form have been made available online though the State of Montana's Court system for those who are seeking a Dissolution of Marriage (also known as divorce) and/or Parenting Plan (also known as custody).  The forms are designed to represent yourself without an attorney, although they cannot substitute for the advice of an attorney and may not be appropriate in some circumstances. If your case is complex or you have minor children, you are highly encouraged to consider retaining the services of an attorney.
